WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission (CPSC) is once again urging all parents and caregivers to immediately stop using convertible "close-sleeper/bedside sleeper" bassinets manufactured by Simplicity Inc., of Reading, Pa. CPSC has learned of two additional infant deaths since the August 2008 safety alert and recall announcements that notified consumers of two infants who had strangled in their Simplicity bassinets. To date, CPSC is aware of four infants who have died in the close-sleeper bassinets.

CPSC learned that in September 2008, a two-month old female in Demorest, Georgia suffocated when she rolled into the adjustable fabric siding of the bassinet, pressing her face into a "pocket" which formed near the Velcro© fasteners. In another incident in January 2009, a six-month old female in Fort Worth, Texas appears to have been fatally entrapped in the bassinet’s bar opening created when the Velcro© fasteners were not secured. An investigation into this infant’s death is ongoing.

In addition, CPSC has received two separate reports involving a 10-week old and a 3-month old whose heads became entrapped between the lower bassinet bar and the mattress support. The infants were freed by their caregivers without injury.

The Simplicity 3-in-1 and 4-in-1 convertible bassinets, including bassinets with the Graco logo and “Winnie the Pooh” motif licensed by Disney Consumer Products, contain metal bars that are covered by an adjustable fabric flap. This fabric flap is secured by Velcro© fasteners, which are intended to be partially removed when the bassinet is converted into the bedside/close-sleeper mode. If the Velcro© fasteners are not properly re-secured when the fabric flap is adjusted, or if the consumer entirely removes the fabric flap exposing the metal bars, an infant can slip through the opening between the lower metal bar and the mattress support and become entrapped and suffocate or strangle. Consumers are urged to check their bassinets to ensure they are not using one of the recalled Simplicity bassinets.

Parents and caregivers are advised to return these bassinets to the retail store where they were purchased. All of the recalled bassinets can pose a danger to infants.

To determine whether your bassinet has been recalled, please check for the model number and the manufacture date code pictured below. Bassinets manufactured prior to 21st week of 2008, “2108” (before May 18) are recalled. The model numbers for recalled bassinets manufactured prior to “2108” are:

SFCA Inc., the Reading, Pa.-based company that purchased the assets of juvenile product manufacturer Simplicity Inc. after foreclosure, no longer appears to be conducting day to day operations. SFCA Inc. is no longer answering phone calls or responding to e-mails from consumers.




FOR IMMEDIATE RELEASE -- Fort Washington, PA (November 7, 2008) – Johnson & Johnson • Merck Consumer Pharmaceuticals Company (JJMCP) is voluntarily recalling approximately 12,000 units of Infants' MYLICON® GAS RELIEF DYE FREE drops (simethicone-antigas) non-staining sold in 1 oz. plastic bottles that were distributed after October 5, 2008 nationwide. The company is taking this action in consultation with the U.S. Food and Drug Administration (FDA). Although the potential for serious medical events is low, the company is implementing this recall to the consumer level as a precaution after determining that some bottles could include metal fragments that were generated during the manufacturing process. If any medical events were to occur, most are expected to be temporary and resolve without medical treatment. Parents who have given the product to their infant and are concerned should contact their health care provider immediately.

Description: This recall involves the Munchkin Deluxe Bottle and Food Warmer with Pacifier Cleaning Basket 2-in-1 Design, model #13301 and lot number TP-1487. The product is used to warm food and bottles of various sizes. The set includes a main basket, a lift-out basket, an adapter ring, and a measuring cup. “Munchkin” is located on the front of the warmer and “TP-1487” is located on the bottom. Only warmers bearing lot number “TP-1487” are included in this recall.

Sold at: Various retailers nationwide and company’s catalog from June 2007 through April 2008 for about $20.

Incidents/Injuries: Avon has received 41 reports of the warming polar bear overheating, including six reports of minor burns.

Description: The recalled white plush warming bear has a red hat and brown nose. It is about 17 inches long and comes with a buckwheat-filled pouch that is heated in a microwave oven and is inserted into an opening in the bear’s belly. “Avon Products” is printed on a white tag sewn onto the bear.

Sold exclusively by: Avon independent sales representatives and at www.avon.com from July 2007 through February 2008 for about $13 when sold alone, or as part of a gift set that sold for about $100.

If more than one magnet is swallowed, the magnets can attract each other and cause intestinal perforations or blockages, which can be fatal.

Incidents/Injuries: MEGA Brands and CPSC have received 19 reports of magnets coming loose, including one report of a 3-year-old boy receiving medical treatment to remove a magnet from his nasal cavity and one report of an 18-month-old boy with a magnet in his mouth, which was not swallowed.

Description: The Magtastik and Magnetix Jr. Pre-school Magnetic Toys include small flexible parts embedded with magnets that allow the parts to connect to large, colored metal balls.

Hazard: The aluminum rings on the sling carriers can bend or break. This can cause the fabric to slip through the rings and infants to fall out of the carrier.

Incidents/Injuries: Ellaroo has received four reports of the rings bending and two reports of rings breaking. No injuries have been reported.

Description: This recall includes Ellaroo Ring Sling baby carriers with item numbers 2101 and 2102 printed on the outside of the product box. The Ellaroo Ring Sling has a fabric carrier with two aluminum rings that is worn by the user to carry an infant up to 35 pounds. The carriers are sold in mahogany, mango stone, brasilia and malay color prints. Only sling carriers with lot numbers 03/07 and 07/04 printed on a label, under the size label, inside the Ring Sling are included in the recall.

Description: The recalled Infantino Lamb Grabby Rattles™ are shaped like a lamb with an Infantino elliptical-shaped logo stamped on the front right foot of the lamb. Only rattles with date code 0907 printed on the back of the left ear of the lamb are included in the recall. The production batch code is printed in a dial format with the year in the middle of a circle and an arrow pointing to the number on the circle that indicates the month. Rattles that do not have a date code are not included in the recall.

Sold at: Wal-Mart, Babies “R” Us and other specialty stores nationwide from September 2007 through February 2008 for between $3 and $4.

Hazard: The cribs fail to meet the federal safety standards for cribs. The four support brackets on the mattress support spring are too long. The brackets prevent the spring from lowering to the full 26 inch minimum height in its lowest position, allowing children inside to crawl over the railing, posing a fall hazard.

Incidents/Injuries: None reported.

Description: The cribs are wooden. The recalled cribs include: Majestic Curved Top cribs with model number 9500; Majestic Flat Top cribs with model number 9000; Essex cribs with model number 7100; Brighton/Sussex cribs with model number 9100 and Captiva cribs with model number 5100. Only cribs with manufacture dates between November 1, 2005 and November 1, 2007 are included in the recall. The crib model number is printed on the white label on the bottom inside of the right side rail. The crib manufacture date is printed on either the white label near the model number or on the white label located on the bottom of the headboard. Cribs with a green sticker on the mattress frame are not included in the recall.
